To achieve an accurate evaluation, READ THE PREPARATION SHEET FOR YOUR STUDY AS SOON AS YOU RECEIVE THE PAPERWORK AND NOTE IT IS IMPORTANT TO REVIEW AGAIN ​SEVERAL DAYS PRIOR TO YOUR APPOINTMENT, 24 HOURS BEFORE, AND AGAIN THE MORNING OF YOUR APPOINTMENT. You should avoid conditions that would cause artificial influences.

On-line case history

Be sure to fill out your patient history forms 48 HOURS prior to your appointment when submitting on line. Remember you can return to this form using your secure link as often as you need. Once submitted you will no longer have access. We will and can make any changes at the time of your appointment as we will review it.

 

Hardcopy paperwork

When using a paper history form be sure to complete and bring the form with you at the time of your appointment. As we have many locations please do not mail the form in.
